Visitor killed by fall from Tokyo roller coaster

A 34-year-old man died Sunday afternoon when he fell about 7-8 meters (20-24 feet) to the ground at the Tokyo Dome City Attractions in Bunkyo Ward.

The Spinning Coaster Maihime, reaches a top speed of 39.6 kph (22.4 mph). Reports say that the man may have been ejected on a turn.

UPDATE there is obviously some manufacturing flaw with this ride this is the second ejection on this ride and it was later deemed that the these two rides are mde by the same manufacturer.
